The $25 tryout fee is a one-time payment that covers all three dates — pay once and attend any or all sessions.

Participation waiver

I, the parent or legal guardian of the player named above, give permission for my child to take part in SheHoops Nation tryouts. I understand basketball is a physical activity that carries a risk of injury. I confirm my child is in good health and able to participate.

In an emergency, I authorize SheHoops Nation staff to seek medical treatment for my child if I cannot be reached. I release SheHoops Nation, its coaches, staff and volunteers, and Bunnell High School from liability for injuries that occur during normal participation, to the extent allowed by law.

I give permission for photos or video taken during tryouts to be used by SheHoops Nation for team and promotional purposes.
